In an effort to go paperless, we are developing an electronic family/household information form that will be pre-filled with current information from our database. This will enable parents to more easily enter and/or update information related to emergency contacts, transportation and the like.
Some required forms, such as the physical exam, allergies and conditions and allergy action plan, will still need to be downloaded, printed and returned to the Lower School Office or the Middle/Upper School Office.
The 2012-2013 Physical Exam Form is below, and other forms will be posted by June 1.
