EMERGENCY CONTACT INFORMATION
Please log into AIS-On and review/update/revise your contact information in the system no later than August 1, 2018. Please click here for an instructional video on how to complete this task.
All forms listed below are posted in AIS-On and need to be received by AIS no later than Wednesday, August 1, 2018. Please send completed forms via email to Jeannine Kelly RN BSN for Middle/Upper School or Lorraine Lampe RN for Lower School; fax(610-527-8129); or mail (The Agnes Irwin School, 275 S. Ithan Ave., Rosemont, PA 19010, Attention: Health Center). Parents, please keep a copy of all documents.
PHYSICAL EXAMINATION FORM
A current Physical Examination Form is required for all new students. Please note, the physician-signed physical examination form is required on a rolling basis. For example, if a student has a wellness checkup in March, then an updated physical form would be due the following March. If you have forwarded the physical on a rolling basis and are confident that it is on file, you do not need to forward an additional copy.
A current Dental Form is required for all new students and all returning students in grades PreK, K, 3, and 7.
IMPORTANT: IMMUNIZATION REQUIREMENT
The PA Department of Health now requires that all students entering 12th Grade receive a second dose of meningococcal conjugate vaccine (MCV). Please discuss this with your child's doctor. For a complete list of required immunizations for school attendance in PA, please click here.
STUDENTS WITH MEDICAL CONDITIONS
The following medical conditions: anaphylaxis, seizures, asthma, and diabetes, require action plans/care plans to be completed by your child's physician and kept on file with the school nurses. These forms must be renewed annually and on file in the Health Center prior to the start of the school year.
MEDICATION ADMINISTRATION FORM
All students who need to take prescription medication or over-the-counter medication other than Tylenol, Ibuprofen, Benadryl, and Tums during the school day must have a Medication Administration Form signed by the physician and parent/guardian on file with the Health Center. Emergency medications including insulin, rescue inhalers, and EPI Pens, can be carried and self-administered by Middle and Upper School students only with a Medication Administration Form on file and applicable Action Plan.
Tylenol, Tums, Ibuprofen, and Benadryl are the only medications that the school nurse may administer to your child during the school day on an as needed basis, but parents/guardians must list the medication of preference in the "medical" section in AIS-On. LOWER SCHOOL DISMISSAL FORM
Lower School parents are required to log into AIS-On and complete the online dismissal form (look for the yellow banner) by Monday, August 27.
UPPER SCHOOL PRE-SEASON DATES AND REQUIREMENTS
Preseason for Upper School cross country, field hockey, soccer, tennis, and volleyball will begin on Monday, August 21. All student-athletes are expected to attend preseason, and practice schedules and competition schedules can be found on the school website in the Athletics section. Middle School fall sports will begin on Thursday, September 7.
As a reminder, the following requirements must be met prior to any and all participation in athletics programs for students in grades 6-12:
- Each student-athlete and a parent or guardian must complete Concussion and Sudden Cardiac Arrest education each year. The Concussion and Sudden Cardiac Arrest Presentation for 2018-19 will be available on AIS-On at the end of July. Parents/guardians and student-athletes will receive an email when the presentation is available online.
- A current physical examination, one year from the date of the last completed physical (on a rolling basis), must be on file with Jeannine Kelly, the Middle/Upper School nurse.
